Documentation Responsibilities for Service Termination by Members

- Membuat pemeriksaan kesihatan di Rumah Sakit Angkatan Tentera (RSAT) dengan membawa Borang Angkatan Tentera (BAT) D 70 - Bertukar Ke Pasukan Simpanan dan BAT B 83 - (Pemberhentian Askar Laskar).
- Prepare a passport-sized photo.
- Apply for a National Identification Card at JPN by bringing the Soldier’s Retirement Confirmation Letter (Appendix 1 to D Duplicate - ARKTUJA No.3).
- Prepare the following personal documents:
- Copy of the Marriage Certificate.
- Copy of the Wife's Identification Card.
- Copy of the Children's Identification Card/Birth Certificate.
- Copy of the Bank Account Book/Bank Statement.
- Complete the Unit Settlement Certificate.
- Fill out and obtain bank verification for the Pension Payment Scheme (SPB) 1 form.
- Complete all income tax matters with the Inland Revenue Board (LHDN) independently to ensure that the Tax Clearance Certificate (SPC) is issued before service termination/retirement. This is to avoid delays in retirement benefits or LTAT contributions.
Documentation Responsibilities for Discharge by the Unit
- The unit must inform the member about the termination of service.
- The unit is required to take the following actions:
- Provide the Soldier’s Retirement Confirmation Letter (Appendix 1 to D Duplicate ARKTUJA No. 3) to the member for applying for a National Identification Card at JPN.
- Prepare Form BAT D 70 – Transfer to the Reserve Unit and Form BAT B 83 – Discharge of Soldier.
- Schedule a health check appointment for the member.
- Ensure the member completes the Unit Debt Settlement Certificate.
- Complete Form BAT D 191 – Member’s Leave Book.
- Ensure the member prepares the following:
- Passport-sized photo.
- Complete the SPB1 form.
- Copy of the Bank Account Book/Bank Statement.
- The unit must ensure that all the above matters are fully prepared and carried out by the member. All these documents must be submitted to JRP along with the member’s personal documents no later than 12 months before the Service Termination Date (TTP).
